چکیده

OBJECTIVE/HYPOTHESIS This study aimed at investigating the correlation between a battery of diagnostic symptoms of definite Meniere's disease (MD) and the degree of endolymphatic hydrops (EH) in the inner ear. STUDY DESIGN Prospective study. METHODS Fifty-four patients diagnosed with unilateral definite MD were enrolled in the study. The hearing levels of all patients at the low, middle, and high frequencies were evaluated. Three-dimensional fluid-attenuated inversion recovery (3D-FLAIR) and three-dimensional real inversion recovery (3D-real IR) magnetic resonance imaging (MRI) were performed 24 hours after bilateral intratympanic injection of gadolinium to assess the presence and grading of EH. RESULTS Various degrees of EH were observed in the vestibule and/or each turn of the cochlea in the affected ears of all patients. The duration of MD disease and low-tone and middle-tone hearing thresholds were proportional to the extent of EH in the vestibule and cochlear. However, no significant correlation was demonstrated between EH and other aspects of symptoms such as high-tone hearing loss, tinnitus, and aural fullness. Of all subjects, 16.7% exhibited bilateral EH on MRI exam who were diagnosed with unilateral MD based on diagnostic criteria. CONCLUSIONS EH in the inner ear of MD patients exhibits a progressive deteriorative trend over time. Low-tone and middle-tone hearing thresholds can indirectly reflect the severity of EH in the cochlea. EH may not be the primary cause of tinnitus and aural fullness in patients with MD. Patients diagnosed with unilateral MD should maintain long-term regular follow-ups for the possibility of developing bilateral EH. LEVEL OF EVIDENCE 4.

Rapid Communication Endolymphatic Hydrops in the Horizontal Semicircular Canal: A Morphologic Correlate for Canal Paresis in M enière’s Disease

INTRODUCTION For almost 70 years, since Yamakawa and Hallpike and Cairns first discovered endolymphatic hydrops as the morphological hallmark of Menière’s disease, it has only been possible to visualize endolymphatic hydrops (ELH) on histologic staining of postmortem temporal bone specimens. In a landmark article, Nakashima et al. were the first to succeed in visualizing ELH in living Menière’s...

Two-phase endolymphatic hydrops: a new dynamic guinea pig model.

The classical guinea pig model for Meniere's disease, in which endolymphatic hydrops was achieved by destruction of the endolymphatic sac and obliteration of the endolymphatic duct, is a non-physiological profound model with shortcomings in relation to Meniere's disease as seen in patients. We developed a more subtle animal model; the two-phase endolymphatic hydrops. This model is based on a co...

Longitudinal up‐regulation of endolymphatic hydrops in patients with Meniere's disease during medical treatment

Objective/Hypothesis Meniere's disease (MD) is a common inner ear disease characterized by repeated episodic vertigo, fluctuating sensorineural hearing loss, and tinnitus. Its pathology is defined as endolymphatic hydrops (EH) in the inner ear and EH has been hypothesized to correlate with the clinical symptoms of MD. We presented the dynamics of in vivo EH in MD patients during medical treatme...

Comparison of the diagnostic value of 3 T MRI after intratympanic injection of GBCA, electrocochleography, and the glycerol test in patients with Meniere's disease

CONCLUSION 3 T MRI after intratympanic injection of gadolinium-based contrast agent (GBCA) is more useful for the diagnosis of endolymphatic hydrops compared with the glycerol test and electrocochleography (ECoG). OBJECTIVE To investigate the relationship between 3 T MRI after intratympanic injection of GBCA, the glycerol test, and ECoG in patients with Meniere's disease (MD). METHODS A tot...

Volume Quantification of Endolymph after Intravenous Administration of a Single Dose of Gadolinium Contrast Agent: Comparison of 18- versus 8-minute Imaging Protocols.

PURPOSE We measured the volume of the endolymphatic space by extending a previously proposed less observer-dependent method of area quantification and compared volume measurements obtained using long (18 min, Image A) and short (8 min, Image B) scan times. METHODS We performed MR imaging of 40 ears in 20 patients with clinically suspected endolymphatic hydrops 4 hours after intravenous admini...
